Background Information
Team Dynamix is the Enterprises’ ticket management system used to submit and track tickets relating to Team Member requests. The following Job Aide will show the steps to fill out and submit a Hardware/Software Request Form, formerly known as a BNRF/UARF, in Team Dynamix. These steps detail how to request computer, application, or email access for a Team Member or group of Team Members.
How to Submit a Hardware/Software Request Form
- Navigate to the Team Dynamix client portal: https://service.casinoaz.com/TDClient/279/Portal/Home/
- Select IT/AV Requests.
- Select Software & Hardware Request.
- Fill out and complete the following fields:
- Brief Description of Request
- Ex) Computer & Email Access for Team Member
- Person receiving the hardware or software access (if different from requestor)
- Only one name can be entered for this field.
- Phone Number
- Location – Delivery
- Search and select the Team Member’s main location.
- I am requesting…
- Select either or both of the first two boxes as needed.
- I would like to…
- Select either or both boxes as needed.
- Select Application Access Requested
- Check boxes for all applications the Team Member will need access to.
- If the application is not listed, select Other and list the application in the next field – Application Security/Profile.
- Please list Application Security/Profile level requested or who to mirror access from
- If there is an existing Team Member with existing access you’d like the access to be mirrored from, list their full name in this field.
- Select Network Files, Folders, Email Groups, or VPN Access
- Check boxes for all applicable items that the Team Member will need.
- Please list requested access or who to mirror access from. Include File Location or Group Names
- If there is an existing Team Member with existing access you’d like the access to be mirrored from, list their full name in this field.
- Business Justification
- Enter a business justification for this requested access that will require Manager & Director Approval.
- IMPORTANT: If multiple Team Members will also need this requested access, list out the names of those Team Members in this field. One ticket should be submitted for all applicable Team Members and all access requests.
- Select Submit Hardware/Software Request.
- An email notification from Team Dynamix will be emailed to the Requestor with details regarding the ticket creation, including the ticket ID number.
NOTE: If changes are needed to the ticket, please contact IT Support with the ticket ID number so the ticket can be updated appropriately. Changes include adding application, email or network access or adding Team Members to the original request.
