UKG - HR Termination Forms

Background Information

This Job Aide will explain the method of accessing, modifying, and submitting HR Termination Forms within UKG.

Submitting a Termination Form in UKG

  1. Navigate to UKG and login.     
  2. Select the hamburger menu in the top, left-hand corner to open navigation column.
  3. Select HR drop-down and click HR Actions.
  4. Under the Available tab, scroll down to the Termination section and select Start.
  5. This will open the Termination pop-up window. Search for an employee and select their name.
  6. Select an Effective Date for this form. Effective Dates can be:
    • Current day.
    • Date in the future.
    • CANNOT be a date in the past.
  7. Select Continue.
  8. Within the Termination Form, complete the following fields:
    • Account Status – change to Terminated.
    • Terminated Date.
    • Last Day Worked Date.
    • Termination Reason – select a choice from the drop-down menu. Can change duration to Days, Weeks, etc. in drop-down beside this field.
    • Check box for No Notice if applicable.
    • Add any Termination Notes – if not eligible for rehire at company, please note in this section.
    • Check Rehireable box if employee is eligible to be rehired at company.
  9. Select Continue in the upper, right-hand corner.
  10. Human Resources section – leave blank as HR will complete this section.
  11. Select Submit.

Notes for HR Termination Forms

  • Review the status of a submitted termination form by navigating to HR Actions dashboard (see steps 1-3) and selecting the Submitted tab.
  • Within the Termination Form, you can select Save and return to the form at a later date.
