Policy Acknowledgement Documents
Enterprise policy changes must be acknowledged by all Team Members. Using the UKG application, Team Members can log in and review policies and sign acknowledgement forms. The following steps navigate the process of signing acknowledgement forms in UKG.
Signing Acknowledgement Form
- Log In to UKG and Navigate to the Main Menu using the Hamburger Icon.
- Click the “My Information” dropdown and click “My HR”.
- At the Top of the page, click the “Forms” tab, then click “Other Forms”.
- Under “Open” Forms, click the available form you’re completing.
- Use the Page Navigation arrows in the upper left corner to review the policy. After reviewing the policy, read the acknowledgement form on the last page.
- Be sure to fill in any required fields available. All required fields are identified with an asterisk (*) symbol.
Note* - Some forms require Date fields to be filled and some forms will have required fields filled in for you
- You will be prompted to “Save and Sign” to continue. Click “Save and Sign”.
- Enter the last four digits of your Social Security number in the field provided in the popup prompt, then click “I Agree”.
- When the form has been filled completely, click “Submit” in the upper right corner of the screen.
Note – If you would like to, you may download and save a copy of the form by clicking “Download PDF”.
- You will be prompted to confirm. Click “Submit” in the popup to submit the form. You will receive a popup confirming the process is complete and you’ve submitted the form.
